In that case, breast retroughion or breast retroughion may be considered. This involves removing excess breast tissue and forming a smaller new breast. If the retroughion is limited, local anesthesia and light sedation may suffice. Larger retroughions are routinely done under general anesthesia. Our surgeons always discuss the possible options during the initial consultation.
Technique in breast retroughion
The procedure takes about 2 hours and in some cases requires an overnight stay at our Clinic
Prior to the procedure, the plastic surgeon will mark your breasts according to the treatment plan.
Once the surgeon has made his markings, you will be escorted to the operating room. There, the anesthesiologist and nurse will administer the anesthesia so you will not feel anything from the surgery.
A wedge-shaped section of skin and glandular tissue is removed from the underside of the breast. Then the nipple is moved upward and, if necessary, the areola is reduced in size. In this way, a smaller breast is modeled.
During surgery, the breast tissue is moved up and skin and breast tissue are removed.
Depending on the new position of the breast, a hole is made in the skin into which the nipple is sutured. Suturing is done from the nipple straight down to the inframammary fold. The nipple is reduced in size if necessary.
Once excess skin and tissue are removed, the breast can be rebuilt.
The threads are resorbed by the body itself. The sutures are additionally reinforced with Micropore Tape and an absorbent dressing is placed above them.
Then you will get the support bra on. So you will wake up with the special bra on.
Visibility of scars with breast retroughion
Our surgeon will try to make as few scars as possible. Only the anchor scar is necessary for really extremely heavy breasts where no other option is possible. In most cases, you will have a scar around the nipple and it will run vertically to the inframammary fold. This normally results in a narrow white line.
Around the areola, you can afterwards opt for any nipple pigmentation so that the scars are no longer visible. This can be done at the earliest 4 months after your surgery.
Not only the mammary gland but also the skin around it is removed. This cannot be done without scarring. A breast retroughion always leaves a scar around the areola (which is often also reduced in size).
Depending on the amount of breast tissue removed, a scar will also run vertically from the areola to the breast fold.
It is often possible to avoid or reduce the horizontal scar (anchor scar).
Only when it comes to an extreme breast retroughion, one must also make a scar at the bottom of the breast fold (horizontally) = the classic "anchor-shaped" scar is created (around areola, vertically and horizontally in the breast fold). This will be able to be determined after examination by the surgeon.
Healing scars after breast retroughion
Scars at the level of the breast usually heal quite nicely, but always remain visible to a greater or lesser extent.
Together with your surgeon, scar treatment will be discussed after surgery. The scar around the nipple usually heals nicely, and can be finished with nipple pigmentation in disruptive cases.
Aftercare for breast retroughion
- After the breast retroughion, you will usually stay 1 night in the clinic unless otherwise discussed with the surgeon.
- After surgery, you will be taken to the recovery room. Here your blood pressure and heart rate, among other things, will be monitored.
- You will be here for about a 1.5 hours.
- When all parameters are found to be good you may go to the room.
Day after breast retroughion
You may shower the day after surgery. Be sure to keep the wound dry. The plaster is waterproof and can be patted dry. Leave the plaster on for two weeks (unless soiled).
You must leave the little white band-aids in place at all times (they stick to your wall thread and may only be removed by the surgeon on the day of your follow-up checkup) = 2 weeks after surgery.
What else to think about with breast retroughion:
The first two weeks: do not raise your arms, do not exercise.
The first six weeks: get plenty of rest and no physically strenuous activities - such as in the household, think: vacuuming, cooking or heavy lifting. Also, do not swim or go to the sauna.
For the first year, it is wise to cover the scar from the sun
(full sunblock of 100) .
